Isak, visibly tiring, departed just before the hour mark, and Salah almost restored Liverpool’s advantage when he rattled the post following Florian Wirtz’s neat play.
But as they have already done so often this season, Arne Slot’s side found a decisive answer in the dying moments.
Van Dijk rose highest to meet a corner in stoppage time, powering a header past Oblak to spark wild celebrations from the Kop.
